What are the ramifications if a U.S. president disobeys a ruling by a judge attempting to block or reverse an executive decision — especially if the ruling is later deemed unlawful by the Supreme Court?

The Executive Branch has the right to appeal rulings by judges.  The Supreme Court on the emergency docket has demonstrated a capacity to stay rulings the justices believe illegal with great speed.
