Does the Constitution directly state or even imply that all voting- local, state, national- be conducted using paper ballots? Who decides at which level what kind of ballots will be used in an election? of any kind. Is there any validity to the statement that some hold that balloting by mail and/or electronically increases the likelihood of fraud occurring in any election and could you please explain why? Thank you.

The Constitution allows states to determine how elections are conducted, subject to revision by Congress.  Most states allow local precincts to make decisions, subject to revision by the state legislature or, in some states, the state board of elections.  As a matter of history, there has been no more mail fraud in elections that any other kind of fraud, and for the past hundred years, almost no fraud of any kind.  Both parties watch each other closely.  Fraud is hard to do.
