Is there a formal legal process by which presidents and/or presidential candidates are evaluated to be fit for office from a physical and or cognitive perspective?
The 25th Amendment provides that the President may declare himself or herself unfit or the Vice-President and a majority of the department heads may make that decision, or another body authorized by Congress.  That is it.

Let me suggest based on what I see.  I think President Biden does not have the capacity to communicate his positions well in the modern social media environment.  I do think, based on people I know, that he has the capacity to make policy, when that does not require spontaneous sentence construction (think of a person who has lost their ability to type without numerous typos—like me).  Whether he is fit for office depends on whether you think the contemporary presidential office requires communicating clearly and well on modern social media.
